Please complete our Application Form and return it to The Darling Home for Kids. Each completed admission form will be reviewed by our admissions committee. Once your child’s application has been reviewed, all service date booking will be made through our Clinical Nurse Specialist, Joanne Gallevo. As it is our goal to assist families with meeting all of their respite and palliative care needs, we will endeavour to accommodate requests where possible.
If you are currently connected with a special needs access point within your community, you may be required to access our services through your coordinator within that agency. Please contact your coordinator to discuss further.
Documentation and What to Bring
It is required that you bring your child’s own supplies, enough clothing for the duration of the stay and appropriate supplies and equipment your child will be using (i.e. feeding tubes, formula, feeding pumps, incontinent supplies, etc). All medications must be provided in the original bottle with the correct client name, drug name and correct dosage.

In order to ensure that all information regarding your child's care is up-to-date we asked that you complete the Getting to Know Your Child form once a year.
For the health and safety of all who use our respite services, should your child exhibit symptoms of illness (fever, vomiting, diarrhoea, congestion), you/your emergency contact will be called to take your child home.
